Once successfully discharged, our care doesn’t end when they leave our doors. The heads of our nursing department have been invited to speak in front of healthcare professionals about our best practices at Rutgers University. Through what we call the huddle system, we have been able to increase patient success rates well above our competitors. Alicia Fereno MSN, RN, CDONA, FACDONA, IP-BC—National – Ms. Fereno is the Director of Nursing at the Rose Garden Nursing and Rehabilitation in Toms River, NJ. She has 29 years experience working in long-term care, with at least 24 of which being at Rose Garden. NADONA recently recognized her efforts in 2020 by naming her the National Nurse Leader of the Year.
Our Care Doesn’t End After Discharge
In August 2016, Rose Garden employed Clint Ganzon, a seasoned RN, to ensure successful discharges by making bi-weekly home visits. This was done to help prevent ER visits and re-hospitalizations. In addition, the RN coordinates support services, physician appointments, family education and is available 24/7 to provide guidance and go to the home if needed. 20 months since the program was introduced, the observed successful discharge rate has increased 28%.
